I don't think you can relate Nihei's debut work "Blame" with "BLAME!". It's true that there was a guy named Killy that was a police detective and had a really destructive gun (suspiciously similar to a GBE).
However, most of the events in "Blame" produce immense conflict if taken as part of the same story as "BLAME!".
In my opinion, "Blame" was the raw, basic idea behind "BLAME!". This kind of idea can spark a line of thought that leads to a much more complex plot, by means of a process of creation, selection, and refinement.
Nihei had two years to refine his drawing abilities, and his plot. You have to consider that many things were ripped out. The basic idea is almost always really distorted in the end. The only thing that remains is a guy named Killy with a gun that can blow holes into anything.
If you think about it, it doens´t make sense if Killy was a police detective and already had a GBE. Even Cibo, and the CyberLife Society couldn't build one. You think humans in a world that looks to be in the not-too-distant future could, and that it wouldn't be weird for a police detective to carry one?

Ok, I´m done ranting.
