Regarding the focus, you probably wouldn't notice it if you A) don't use the keyboard to select your buttons, (I don't know anyone that does this unless their mouse isn't working, but Windows is pretty good about providing adequate keyboard access) or B) click and immediately release a button. If you click on a button and, while holding the mouse button down, move the cursor off the button (like you started to click on it but then changed your mind) then you should see the focus appear.
About the titlebars, something like that would be XP-only because it would require themes (or something like Windowblinds on previous versions of Windows) -- it's not something you can do on a per-window basis.
